Federal Family and Medical Leave
Act (FMLA)

FMLA refers to an employee's right for time off (paid and unpaid) due to illness or family care in accordance with the federal Family and Medical Leave Act of 1993, and additional provisions granted by the University.

Certification Form
If you are eligible for FMLA, you will need to fill out a medical certification form. You can obtain a form from the A&BS Human Resources Office.
